Following documents required:

1. Applicant must be present.
2. 2 Passport size Photographs.
3. Original and Photocopy of CNIC.
4. Residence Proof in Swat (Copy of Rent Agreement or Copy of Allotment Letter).
5. After application submition verification process will start.
6. In case of Certificate Renewal same process and documents mentioned above are required.
7. General Police Verification Certificate will be Issued or Denied acordingly.
